Introduction
Ohio Wesleyan University supports the practice of protection for human subjects participating in research.  The following information is provided for you to decide whether you wish for yourself and your child to participate in the present study. You may refuse to sign this form and not participate in this study. You should be aware that even if you do agree that you and your child will participate in this study, you are free to withdraw from this study at any time.  If you do withdraw yourself and your child from this study, it will not affect your relationship with Ohio Wesleyan University.

Purpose of the Study
The goal of this project is to understand how student-athletes view their team culture.  This will include personal perceptions and attitudes toward their role and the positive and negative dynamics of their team and group atmosphere.  Ultimately, this information will help inform what aspects of team culture lead to appropriate and inappropriate individual and team behaviors.

Procedures
If you agree to be a part of the study, you will complete the on-line, anonymous survey.  All of your answers will be private and will not be seen by anyone on your team or school.  Only the investigators listed above will have access to view the summary data. In this study:

  1. You will be asked questions about yourself such as your age, race, gender, graduation year and sport and NCAA division.  This will take less than 3 minutes.
  2. You will be asked about your team culture, e.g. “On a scale from low to high, how much do your teammates tease each other?”. This will take less than 20 minutes.

Risks
You may feel some discomfort answering questions about your team life.  Some of the questions deal with sensitive issues, such as drug or alcohol use or illegal activities, and may be upsetting.  All of your answers are anonymous as we are not obtaining your personal name or specific school or team information.  The website is private and no one other than the investigators will have access to the data.  It is up to you where and when you complete the on-line questions.  To protect your privacy we ask that you make sure you are alone when completing the questions.

Benefits
This study will help the researchers understand how positive team culture is built and negative team culture can be removed.  By participating, you will be contributing toward this knowledge that will support this NCAA sponsored research.

Participant Confidentiality
Information collected is anonymous so there your identity remains completely confidential.

Data Storage and Security
Data is stored in the cloud on a 256 bit encrypted, private site with redundant firewalls and load balancers for reliability.

Refusal to Sign Consent and Authorization
You are not required to sign this Consent and Authorization form and you may refuse to do so without affecting your relationship with Ohio Wesleyan University. However, if you refuse to sign, you cannot participate in this study.

Cancelling this Consent and Authorization
You may withdraw your consent to participate in this study at any time. You also have the right to cancel your permission to use information collected about you in writing, at any time, by sending your written request to: Dr. Jay Martin. If you cancel permission to use your information, the researchers will stop collecting additional information about you. However, the research team may use and disclose information that was gathered before they received your cancellation, as described above.
